If you're new here, OR if you haven't been to the clinic in the last 2 years. Call to schedule your New Patient Visit, which will take about 1 hour. After you book this, you will get an email request to fill out intake form online-- click on the link in that email, and fill out the intake form. Plan to arrive on time, or 20 mins early if you haven't filled out the intake. We have a contactless payment option available on our website www.cas-olympia.com/payment.
